What to put in a travel consent form

A Travel Consent Form is needed if a minor is travelling without parents. There is a lot of information that needs to be included in this form. The child’s complete should be placed off course together with the name of the parents, and the name of the person your child is travelling with as well as relationship to that person. You will also need the written consent stating that you are allowing for the child to travel with that person and it will be necessary to put the medical condition of the child. Some information like birthday, itinerary, place of birth, passport number and birthmarks may also be needed to avoid identity theft.
